QEHB Placement

Paperwork needs to be scanned and e-mailed to Nicola Bradley (nicola.bradley@uhb.nhs.uk), Senior Medical Resourcing Officer QEHB at least 8 weeks before your placement:

• Copy of passport (visa if required)
• 2 proofs of current address (see list below for information on accepted documents).
• Honorary contract application form (please add the correct start and end date of your placement)
• Declaration A form
• Car park application – this is ONLY if you pay for parking at your current NHS Trust.
• Rest of clearances i.e. DBS & Occupational Health will be requested however if you have a DBS dated within 3 years, please send the certificate.
